New Patients:

Appointment Changes or Cancellations:
We ask that any appointment reschedules or cancels be done no less than 48 hour prior to your scheduled appointment.

Before You Come:

• Request records from your prior dermatologist or primary care physician. This can either be done by bringing in your records on the day of appointment, or having the office fax records to us at 315-701-0075.
  • Having records by the day of your appointment will greatly expedite your care.

• Go to the Patient Portal and update your medical history and input your pharmacy.

What To Bring:

• Insurance Card
• Photo ID
• List of medications (or bring your actual medications)

When To Arrive:

Plan to arrive 20 minutes prior to your appointment
